Sage Eldercare services include a broad scale in-home assessment and care plan & implementation and consultation for plan. We begin with a free 1/2 telephone consulation with a care manager. During this consultation we discuss clients history and review any concerns the caller may have. Afterwards, we conduct an in-home assessment and develop a care plan. Our in-home assessment evaluates the client's current situation and any presenting issues, including: medical; psych-social; in-home care and safety; medication; legal; financial and, in some cases, hospital discharge plans. Our care-plan includes a written set of recommendations for the client's ongoing care, based on our evaluation and any background information provided. Our implementation services include but are not limited to: - Advocating to doctors and other healthcare providers on behalf of client - Monitoring status on a regular basis - Providing updates to family member and health professionals - Making home visits and provide telephone reassurance - Preparing for coming home from hospital stays; evaluate alternative living arrangements if needed - Offer ongoing care management service and overall support - Acting as liaison to families at a distance - Reviewing financial, legal and health issues - Recommending, arranging and facilitating increased socialization and/or intellectual stimulation Email: info@sageeldercare.com Phone: 650-652-5642 |
